TRANSCRIPTS:
Request: Transcript information is protected by law and requires written permission
by the student or parent/legal guardian of a student under the age of 18. A signed
form is required for each official request. Forms are available in the guidance
office. Students under the age of 18 are required to have a parent/legal guardian
sign only the first request.
Unofficial: Must be requested and released by the registrar directly to the student
or parent/legal guardian of the student under the age of 18.
Official: Must be requested by students or parent/legal guardians of students
under the age of 18.
Official transcripts must be mailed from our office and bear certification and
signature of our registrar or school official.
Records and transcripts are subject to the Family Educational Rights Act of 1974
and the Illinois School Students Record Act of 1975. A complete copy of these
acts is available for review upon request in the Guidance Office.
Records are released only with prior written consent. Certain individuals or
agencies have limited access to statistical kinds of information in accordance
with strict procedures. Mundelein High School District 120 will also make education
records available under court order or when legally bound by a subpoena. Parents
of eligible students will be notified that such a request is being honored.
Parents and eligible students shall have the right to review, copy, challenge,
object to, and insert explanatory notes in the record. Requests to review records
should be made in writing to the Superintendent.
RECORDS:
A permanent record of the courses taken
by each student containing the grades received
and scores for college entrance tests is
kept in the Registrar’s Office. When a student transfers to another school
or applies for admission to college, the school will forward a transcript of
this record. Forms for the release of the transcripts to individuals or organizations
are available from the Registrars ’ Office upon written request.
Completed college applications and transcript request forms must be submitted
to the student’s counselor for processing. College applications will be
processed and mailed through the guidance office. Student’s applying
to college must allow five (5) school days for the processing of their applications.
