All 9th grade students are required to have a physical examination and complete immunizations (including the Hepatitis B series) before they begin high school. To the right, you can download the physical exam form required by the state of Illinois.
Every effort must be made to have this physical exam completed PRIOR to the start of school in August. An appointment card or fax from the doctor's office confirming an upcoming appointment will be accepted. (The nurse's fax # is 847-949-0599).
Please be aware that this required entry physical exam can be used as a sport physical for one year from the date completed. However, a sport physical cannot be used as the required physical for 9th grade.
We suggest that you make an appointment now for a physical exam - sometime in late spring or early summer.
Any student NOT in compliance with these health requirements will be excluded from school the Tuesday after Labor Day.
